A photocopy of your Computerized National Identity Card, duly attested by a Gazetted Officer.

3. Police Report (Original)
A formal FIR or police report from the concerned police station regarding the loss or misplacement of your documents.

4. Newspaper Advertisement
Publish an advertisement about your lost property documents in two nationwide newspapers — one in Urdu and one in English (approved by DHA’s Media Cell).

- Attach the complete page of both newspapers with your application.

A written application addressed to DHA Lahore requesting the issuance of a duplicate document.
